vCare-Virtual coaching activities for rehabilitation in elderly

In the EU-funded project vCare, an international research group is investigating how a virtual coach can be used to combine health information, data from the living environment and regular interaction with those affected to support the transition from the rehabilitation phase to the home environment.

The focus is on providing these people with personalised rehabilitation. This empowers those affected to live independently even in old age and in this way improve their quality of life. Unfortunately, the continuity of the rehabilitation process often interrupts with the transition to the home environment. Therefore, the vCare project aims to provide a smart coaching solution grounded in the logic of personalised care pathways. It is based on virtual coaching activities to assist elderly people to continue with personalized rehabilitation- adapted to the age-related conditions of the patient.
