Innovative Versorgungsprozesse zeichnen sich durch verschiedene Merkmale aus, die darauf abzielen, die Effizienz, Qualität und Patientenzufriedenheit im Gesundheitswesen zu verbessern. Hier sind einige charakteristische Merkmale solcher Prozesse:
- Patient-centredness: The patient is at the centre of the care process and his/her individual needs, preferences and goals are taken into account.
- Interdisciplinary collaboration: Collaboration between different health professionals and sectors is encouraged. Doctors, nurses, therapists and other experts work closely together to provide comprehensive and coordinated care.
- Use of technology: Technology is used to improve the flow of information, facilitate communication and increase efficiency.
- Prevention and early detection: Preventive measures and early detection of diseases is focused on.
- Continuity of care: Seamless and continuous care across different levels of care is sought. Transitions between outpatient and inpatient care, rehabilitation and aftercare will be improved to ensure that patients receive the right care at the right time.
- Data-based decision-making: Existing data and analytics are used to make informed decisions and improve care. By collecting and analysing data, patterns and trends can be identified to develop better treatments and strategies.
At AIT, we support medical providers and caregivers in developing innovative digitally-enabled care processes. The KIOLA® framework offers various modules (Disease Plugins) as well as configurable core components (Core Modules) and telemonitoring components (Care Modules), which can be combined to create customised solutions for integrated care. By configuring Careplans, the telemedical treatment of different diseases can be optimally supported in several intensities.

Survivorship Passport - SUPA
The aim of the entire project portfolio is to develop a comprehensive technical infrastructure for a Survivorship Passport (SUPA) as a basis for the follow-up care of successfully treated patients (survivors) after a cancer illness, a severe haematological or immunological illness in childhood or adolescence, as well as those who are at risk of developing a cancer predisposition syndrome. Individually suggested follow-up recommendations based on the therapy of the oncological disease should enable optimised health follow-up and health care in adulthood. In this sub-project, the existing ELGA Outpatient Clinic Report is to be adapted to the requirements of a Surviorship Passport in order to serve as a hub for intersectoral data exchange in the future.

TELEMED5000 | The main objective of the overall project is to develop a system solution that makes REMOTE PATIENT MANAGEMENT (RPM) technically possible for large numbers of patients in regular care. DemiCare | Living with dementia is a challenge for both patients and their family members. Gesundheitstourismus - Tele-Prävention
| The aim of the research project is the exemplary development of a tele-prevention programme used in tourism (for the Standortagentur Tirol). In addition to the organisational and legal framework conditions, the technical possibilities in health tourism facilities are to be tested. The results of this piloting are also to be included in the development of a concrete health tourism offer. PanCareSurPass | PanCareSurPass will conduct a robust assessment of SurPass implementation by first conducting a preliminary study in six countries representing the three manifestations of health systems in Europe (Austria, Belgium, Germany, Italy, Lithuania, Spain). This will include a technological assessment to develop the current SurPass IT infrastructures. Ethical, structural, organisational, economic, national, local, data protection issues, health systems and specific national circumstances will be considered.
